To obtain an official Davis College transcript, download and fill out the Transcript Request Form. Fax 419.473.2472 or email the completed form along with your payment.

Please note the following:
It takes 5-10 working days to issue transcripts after the request is received. Transcripts for students who attended before 1983 may require longer to process. This time can vary depending on the time of the year the transcript request is received. Transcripts can be ordered by fax if you are paying the fee via credit card; however, official transcripts cannot be faxed to the receiving party. Transcripts cannot be released if you have an outstanding financial obligation to the college or you have defaulted on a Perkins Loan (NDSL).
Davis College maintains articulation agreements and credit transfer partnerships with other colleges and universities. These agreements allow Davis College students/graduates to advance their educational goals.
